PAFCPIC “WE CARE” REQUIREMENTS FOR Php 100,000 KIA PERSONNEL CLAIMS 1. NSO Death Certificate of the deceased member or Certified True Copy from the Original issued by City Civil Registrar 2. NSO Marriage Contract (if Married) 3. Certificate of Legal Beneficiary from Bn S1 4. Affidavit of Undertaking to be issued by the claimants 5. Certification from Barangay Official (for Associate Member), Unit Commander and /or survivor officer as to the fact of the member’s death 6. Proof of Coop membership of the deceased member (Original copies of Passbook, Coop ID, Share Certificate/s) Note: 1. You can claim from any PAFCPIC Branch 2. All requirements must be “Original Copy”
