County Facts: Square Miles: 1,576 • 30th largest in CA • 31% of residents live in the unincorporated areas of the County Population as of 2012: 491,829 • 21% under the age of 18 • 25% Latino
Services for Children Ages Birth to Five years old & their Families • One phone call to Navigator to screen & direct families to appropriate care, including mental health, developmental services, medical care, social services needs • Collaboration of 4 Local CBOs expert in working with young children and families • Free evidence based Triple P – Positive Parenting Program training to ANY provider who works with children or families
• Free training to family practice providers in evidence based social/emotional screening
School Based Services Sonoma County Office of Education sponsored Student Assistance Project: Project SUCCESS: • 6 school districts – 17 high schools • Partners: • 5 Local CBOs for mental health services • NAMI – Sonoma County
Services for Seniors The Sonoma County OLDER ADULT COLLABORATIVE (OAC) A six agency collaborative, dedicated to the Prevention and Early Intervention (PEI) of depression and suicide in the senior population. Service delivery is achieved by providing culturally competent, case management and mental health services to individuals residing in Sonoma County.

Crisis, Assessment, Prevention, Education (CAPE) Team Serves college and high school students; provides MH training to teachers, parents, students.
Psychiatric Emergency Services (PES) / Crisis Stabilization (5150 Designation) Available 24/7 Accepts referrals from law enforcement & ERs
Mobile Support Team (MST) Meets law enforcement in the field and provides crisis intervention and 5150 evaluations. NAMI and consumers involved with this program. Available: 2 pm to Midnight/7 days per week
Crisis Residential Available 24/7 – 10 Beds Up to 30 days stays
