Nomination Form for the AICPA Outstanding CPA in Government Career Contribution Award ABOUT THE AICPA OUTSTANDING CPA IN GOVERNMENT CAREER CONTRIBUTION AWARD The Outstanding CPA in Government Career Contribution Award recognizes the significant contributions to the CPA profession via government service at the local, state or federal level over a candidate’s entire career (minimum of 20 years). Career Contribution Award candidates will have demonstrated exceptional leadership, high ethical standards and a track record of professional excellence. This award gives the AICPA the opportunity to positively recognize the lifetime achievements of an outstanding CPA in government. Nominations for the annual award can be submitted by the State Societies, members at large, federal, state, or local government agencies, or other professional associations. Self-nominations are not accepted. The award will be presented at the AICPA’s National Governmental Accounting and Auditing Conference (GAAC) East in Washington, DC on August 10, 2015.

Please include sustained activities, events, publications and other outcomes as well as personal attributes that reflect the individual’s impact on the profession.

Please provide a summary of the nominee’s contributions to state and/or national professional organizations, and the impact of those contributions.

Please detail any additional examples of service and leadership that the individual has demonstrated outside of their employer or professional organization.

NOMINATION ACCEPTANCE   

The Government Performance & Accountability Committee Awards Subcommittee will review each nomination to ensure that it meets the awards criteria. Once accepted, individual nominees will be contacted and informed of their nomination status. The nominee will then be required to obtain items in the “Other Nomination Materials” section below.

NOMINEE SUBMISSION - OTHER NOMINATION MATERIALS Letters of Recommendation (Minimum of two required) •



At least one letter of recommendation from the candidate's government employer and/or other government official knowledgeable of the candidate’s accomplishments -- AND-At least one letter of recommendation from the nominee’s State CPA Society, or other professional association, or an AICPA or State CPA Society member. Please note: Letters of Recommendation must contain all of the following criteria: 1. Describe the significant accomplishments that best characterize the nominee’s contribution to the CPA profession during the span of their career in government service. 2. Nominee’s contributions to state and/or national professional organizations, and the impact of those contributions. 3. Nominee’s demonstrated leadership and service to the profession outside of the related government organization.

NOMINEE SUBMITTAL PACKAGE The nominee is required to ensure submission of the nomination package. This includes at least two letters of recommendation and their resume by Friday, May 29, 2015 via e-mail (preferred) or U.S. Mail as listed below: •

Please Note: Self-nominations are not accepted. The Outstanding CPA in Government Career Contribution Award nominations may remain “active” for three years. The nominator must decide each year whether to re-nominate or withdraw their active nomination. If a candidate is not selected to receive an award during their three-year period of eligibility, the candidate’s application is automatically withdrawn. Eligibility for nomination may be restored after a one-year hiatus. Nomination Form for the AICPA Outstanding CPA in Government Career Contribution Award SELECTION PROCESS •

Individual members of the AICPA Government Performance and Accountability Committee and employees of the AICPA are not permitted to nominate a candidate.



Nominations will be accepted based upon fulfillment of the requirements by the Awards Subcommittee of the AICPA Government Performance and Accountability Committee.



Complete nomination packages will be approved for completion by the Awards Subcommittee and forwarded to an independent panel of judges



The winners will be selected by an independent panel. The panel will be composed of three distinguished individuals from each level of government who are knowledgeable of the accounting profession.
